Why this matters

On the historical record this institution saved more lives than any treatment — and it is not a treatment, a drug or a hospital. It is an office with three things: the authority to require a change, the information to know where to require it, and a duty to act that does not depend on who is affected. Clean water, drainage, food inspection, quarantine and vital registration together produced the largest sustained gain in life expectancy ever measured, and almost all of it landed before antibiotics existed. The reason it needs to be an institution rather than good practice is that every one of those actions imposes a cost on someone specific in order to benefit everyone diffusely, which is precisely the decision individuals reliably do not make.

The shape of it

  1. Register births and deaths, with cause and place. Everything else depends on this. You cannot see an outbreak, judge a water supply or evaluate any intervention without it — and the register is cheap, which is why its absence is always a choice.
  2. Map deaths onto the settlement. Clustering by street, well or trade is what converts a list into a cause. This is how the Broad Street pump was identified, and the method long predates knowing what cholera was.
  3. Write the powers down — inspection, condemnation, closure, quarantine — with their limits, and put them in the code from published rule. Undefined public health powers are either useless or abusive, usually in turn.
  4. Make the inspector's position secure. An inspector who can be dismissed by the person being inspected is a formality with a salary.
  5. Act on water and waste first, then food, then housing density. That is the order of return, and it does not change.
  6. Publish the figures, including the bad ones. A register whose numbers are managed stops being an instrument and becomes a claim.
  7. Quarantine is a serious power — it must be time-limited, reviewable, and supported, or it is evaded and becomes worse than useless.

Doing it, step by step

The summary above says what has to happen. This says how, with the numbers that decide whether it works.

1 · Register every birth and every death, with cause and place

Everything else on this page depends on this, and it costs a book and an hour a week.

One register, one format, one person responsible: name, age, sex, date, place — street or well or household — and cause as best known. Deaths within 3 days, births within 7. A standing rule that burial requires registration is what makes the record complete, and completeness is the whole value. Total weekly by cause, and keep the books permanently. See the durable record.

✓ Check: Last month's deaths can be counted by cause and by street from the register alone, and the count matches what the burial party did.

2 · Map the deaths onto the settlement

A list becomes a cause when it becomes a map. Mark every case as a dot at the household on a plan of the settlement — see plot, street and block — and look for clustering by street, by well, by trade, by workplace.

Then divide by the number of people served: 20 deaths on a street of 400 is five times the rate of 20 on a street of 2,000, and without the denominator you will act on the wrong street. John Snow identified the Broad Street pump this way in 1854, decades before anyone knew what cholera was — the method does not require knowing the organism, only counting carefully.

✓ Check: A current map exists with a dot per case and a population figure per street, and the highest-rate street is named rather than the highest-count one.

3 · Write the powers down, with their limits

Undefined public health powers are either useless or abusive, usually in turn. Put into the code from published rule, each with a stated limit and an appeal:

  • Inspection of any dwelling, well, market or workshop, on notice except where delay defeats the purpose.
  • Condemnation of a well, a food stock or a building, with the test that must be met to reopen it.
  • Closure of a trade or a workplace, for a stated maximum period before review.
  • Quarantine of a person or household — see entry 7.
  • A duty to act that does not depend on who is affected, which is what stops the powers being used only downhill.
✓ Check: Each power is written with its limit, its duration and who hears an appeal — and one has been exercised and survived a challenge.

4 · Make the inspector's position secure

An inspector who can be dismissed by the person being inspected is a formality with a salary.

So: appointed by the whole assembly, not by the wealthiest household or the trade being inspected; dismissible only for stated cause, by the same body, in the open; paid from general funds rather than from fees collected from those inspected; and required to report publicly on a fixed interval whether or not there is anything to report. Forbid the inspector from holding an interest in any trade they inspect, and write that down too.

✓ Check: The inspector has condemned something belonging to one of the settlement's most powerful households, and is still in post.

5 · Act on water and waste first, then food, then crowding

That is the order of return and it does not change. Spending on clinical care while the water is unsafe is comforting, expensive, and an order of magnitude less effective.

  1. Water — protect every source, close the ones that cannot be protected, and check the ones in use. See making water safe.
  2. Waste — latrines sited 30 m from water and downhill, emptied and maintained; standing water drained; animals kept off the drinking supply.
  3. Food — market inspection, condemnation of spoiled stock, clean water at food stalls.
  4. Crowding — sleeping density and ventilation, which drive tuberculosis and every respiratory epidemic.
✓ Check: This year's spending and labour, written down, went in that order — and the water and waste items are finished rather than started.

6 · Publish the figures, including the bad ones

A register whose numbers are managed stops being an instrument and becomes a claim, and it never recovers — everyone who might act on it learns to discount it.

Post the weekly totals where people pass: births, deaths, deaths by cause, and the worst street with what was done about it. Publish the failures — the outbreak that was missed, the well that was reopened too early. Never revise a published figure without printing both the old number and the reason, and let anyone inspect the register itself.

✓ Check: This week's totals are posted publicly, last year's are still available, and someone outside the office has checked a published figure against the register.

7 · Treat quarantine as the serious power it is

Quarantine is evaded when it is unsupported, and an evaded quarantine is worse than none because it hides the cases.

So it is time-limited to the disease's incubation period — written per disease — reviewable by someone other than the person who imposed it, and supported: food, water, fuel and wages delivered to the household, or people will go out for them. Write the case definition before the outbreak, keep a line list of who is affected with dates and contacts, and lift the order publicly on the stated day.

✓ Check: A quarantine has run to its written end date with supplies delivered daily and no household leaving it to get food.

8 · Run an outbreak to a written protocol

Outbreaks are decided in the first days, when the people involved are frightened and improvising. Write the protocol while nothing is happening:

  1. Case definition — symptoms, place, time — fixed in writing before counting.
  2. Line list — every case with date of onset, place, contacts.
  3. Curve and map — cases per day, dots per household. The shape tells you whether it is one common source or spreading person to person.
  4. Source control — close the well, condemn the food, isolate the cases.
  5. Review after it ends, in public, naming what was slow.
✓ Check: The protocol exists on one page, everyone in the office can name their part in it, and the last outbreak's line list and curve survive in the records.

How this fails

No vital registration, which makes every judgement about health anecdotal.
Powers assumed rather than written, which collapses at the first serious challenge.
An inspector answerable to the inspected.
Treating clinical care as the priority over water and waste — comforting, expensive, and an order of magnitude less effective.
Published figures that are curated, which destroys the register's value permanently and in one step.

How you know it worked

VerificationYou can produce last year's deaths by cause and by street, and name the action taken on the worst street.
